Military Addresses- How to put in an APO, FPO or DPO address

Is this a military address? going to an FPO, APO, or DPO? 

If so the format should be as follows: 

Country= USA

State= 2 letter abbreviation (ie. AA, AE or AO)

City= 3 letter abbreviation (ie APO, FPO, DPO)

So the address would look like this: 

Smith, James PFC

Unit XXXX, Box XXXX

APO, AP 94332
